Assess the strengths and concerns of your community

A community assessment was conducted and it identifies some strengths and concerns in the community. Eight strengths were identified in the assessment. Firstly, there are comprehensive health resources in place to offer medical services to persons who are sick within the community or refer them to the appropriate facilities outside the community. Secondly, all members of the community have access to basic education. Thirdly, minors have access to basic health care services. Fourthly, early prenatal services are available to pregnant women within the community. Seventhly, community initiatives have successfully encouraged the community members to adopt healthy behaviors that include eating healthy foods and avoiding drugs. The assessment identified eight concerns in the community. Firstly, not all members of the community have health insurance coverage. Secondly, medical care is not available for all members of the community. Thirdly, there are cases of minors being subjected to abuse. Fourthly, healthy foods are not affordable or available for all members of the community. Fifthly, there are many cases of drunk driving within the community. Sixthly, limited availability of affordable and safe housing. Seventhly, preventive medical screenings and dental care is not available to all members of the community. Finally, the community does not recognize or treat psychiatric problems.

Identify potential barriers to implementing community health plans in your community and brainstorm ways of addressing these barriers

The local community can be described as a low-income community. Given this designation, any intention to implement health plans within the community is likely to face some barriers. Firstly, the low incomes makes it difficult for the community members to pay for health programs and services. This barrier would be addressed through soliciting funding partners for the health plans. Secondly, there are unique social and cultural norms that influence health behavior. This barrier would be addressed by conducting health education where community norms are reconciled with healthy behavior. Finally, low population densities that do not match the economies of scale coverage for community health plans. This would be addressed by sourcing funding partners and implementing the essential aspects of the plan to improve cost management. It is notable that the three identified barriers offer context for the needs of the community as a low-income community with the awareness that the strategies to address the barriers must focus on incomes (Allen & Spitzer, 2015).
